Role Context
The Consultant Cardiac Transplant Physician will provide specialist leadership for patients with advanced heart failure who may require heart transplantation or mechanical circulatory support. The role spans candidate assessment, transplant listing decisions, peri-operative management, rejection surveillance, immunosuppression adjustment, infection prevention, and long-term graft care. Working closely with transplant surgeons, cardiac intensivists, anesthesiologists, coordinators, and allied specialists, the consultant will contribute to safer transplant pathways, stronger clinical outcomes, and continued development of advanced cardiac services.
Key Responsibilities
* Evaluate patients with advanced or refractory heart failure and determine appropriate treatment pathways.
* Conduct comprehensive pre-transplant assessments, including clinical risk evaluation, comorbidity review, and suitability for transplant listing.
* Participate in multidisciplinary transplant selection meetings and contribute specialist recommendations regarding candidate eligibility.
* Coordinate medical care before, during, and after cardiac transplantation.
* Manage post-transplant immunosuppression regimens according to patient risk, clinical response, and established transplant protocols.
* Monitor for acute and chronic rejection using clinical assessment, imaging, laboratory testing, biopsy findings, and other appropriate diagnostic tools.
* Develop strategies for infection prevention, prophylaxis, early diagnosis, and treatment in immunosuppressed transplant recipients.
* Oversee long-term graft surveillance and management of transplant-related complications.
* Assess patients for mechanical circulatory support, including left ventricular assist devices and extracorporeal membrane oxygenation.
* Work collaboratively with cardiac transplant surgeons, intensivists, anesthesiologists, heart failure specialists, transplant coordinators, pharmacists, nurses, and rehabilitation teams.
* Manage complex cardiovascular and systemic complications associated with advanced heart failure and transplantation.
* Maintain complete transplant records, treatment plans, consent documentation, surveillance findings, and follow-up reports.
* Contribute to transplant program quality improvement, clinical governance, audit, morbidity reviews, and outcome monitoring.
* Participate in research, teaching, departmental education, and mentoring of physicians and trainees.
* Apply internationally recognized transplant and heart failure guidance, including ISHLT and ACC-AHA standards, in clinical decision-making.
Ideal Profile
* MBBS, MD, or equivalent medical qualification from a recognized institution.
* Board certification in Cardiology or Internal Medicine with formal subspecialty training in Advanced Heart Failure and Cardiac Transplantation.
* Fellowship training completed at a recognized international cardiac transplant center.
* Minimum 3-5 years of post-fellowship experience in cardiac transplant medicine.
* Strong clinical background in advanced heart failure, transplant candidate assessment, immunosuppression, rejection monitoring, and post-transplant care.
* Experience evaluating patients for LVAD, ECMO, or other forms of mechanical circulatory support.
* Detailed understanding of international transplant standards and evidence-based clinical guidelines.
