Key Responsibilities
* Lead the clinical management of cardiac transplant patients, including pre-transplant evaluation, eligibility review, listing decisions, and multidisciplinary case discussions.
* Assess patients with end-stage heart failure and determine suitability for heart transplantation, LVAD support, ECMO care, or alternative advanced treatment pathways.
* Coordinate peri-operative transplant care with cardiac surgeons, anesthesiologists, intensivists, transplant coordinators, nurses, and allied health teams.
* Manage post-transplant follow-up, including immunosuppression planning, rejection surveillance, infection prevention, graft monitoring, and long-term recovery support.
* Interpret advanced cardiac imaging, invasive hemodynamic studies, endomyocardial biopsy results, laboratory markers, and transplant-related diagnostic findings.
* Participate in transplant selection committees and support decisions that align with clinical ethics, patient safety, national regulations, and international transplant guidelines.
* Provide expert consultation for complex heart failure patients requiring advanced medical therapy, device support, or referral for transplant consideration.
* Lead transplant program audits, morbidity and mortality reviews, quality improvement work, and clinical pathway development.
* Mentor fellows, residents, nurses, transplant coordinators, and junior physicians involved in advanced heart failure and transplant services.
* Participate in on-call coverage for advanced heart failure, urgent transplant assessments, peri-operative issues, and post-transplant complications.
* Support academic work, research projects, clinical trials, and evidence-based service development in cardiac transplantation and advanced heart failure care.
Ideal Profile
* Medical degree such as MBBS, MD, or equivalent from an accredited institution.
* Board certification in Cardiology or Internal Medicine with subspecialty training in Advanced Heart Failure and Cardiac Transplantation.
* Fellowship training from a recognized international cardiac transplant or advanced heart failure center.
* Minimum 3-5 years of post-fellowship experience in cardiac transplant medicine.
* Strong experience managing heart transplant recipients, end-stage heart failure patients, LVAD cases, ECMO-supported patients, and complex cardiac care pathways.
* Sound knowledge of immunosuppression protocols, rejection monitoring, infection prevention, transplant eligibility, and long-term graft surveillance.
* Familiarity with international transplant guidelines, including ISHLT and ACC-AHA standards.
* Ability to work confidently in multidisciplinary transplant committees and high-acuity hospital environments.
* Strong leadership, communication, teaching, and clinical decision-making skills.
* Experience in research, audit, academic medicine, or transplant program development is an advantage.
