Role Context

The Consultant Cardiac Transplant Physician will provide expert clinical care for patients with advanced heart failure and those being considered for heart transplantation or mechanical circulatory support. The role is central to safe transplant decision-making, pre-operative optimization, post-transplant recovery, rejection prevention, infection control, long-term graft survival, and coordinated care across a complex tertiary cardiac program. Working with cardiac surgeons, intensivists, anesthesiologists, transplant coordinators, cardiologists, nurses, pharmacists, and allied healthcare teams, the consultant will support high-quality outcomes across every stage of the transplant pathway.

Key Responsibilities

* Evaluate and manage patients with advanced heart failure, end-stage cardiac disease, and heart transplantation needs.
* Conduct pre-transplant assessments, transplant listing evaluations, clinical risk reviews, functional status assessments, and multidisciplinary suitability discussions.
* Provide post-transplant follow-up care, including graft function monitoring, medication adjustment, complication review, and long-term patient surveillance.
* Manage immunosuppression protocols, rejection surveillance, infection prevention, and post-transplant risk reduction strategies.
* Assess patients for mechanical circulatory support options, including LVAD, ECMO, bridge-to-transplant therapy, and destination therapy where appropriate.
* Interpret cardiac imaging, hemodynamic data, laboratory investigations, biopsy findings, immunologic studies, and transplant-related diagnostic results.
* Collaborate closely with cardiac surgeons, intensivists, anesthesiologists, electrophysiologists, transplant coordinators, pharmacists, nurses, rehabilitation teams, and other clinical specialists.
* Participate in transplant selection committees, case conferences, multidisciplinary rounds, morbidity reviews, quality improvement work, and clinical governance activities.
* Support peri-operative management of cardiac transplant patients through close coordination with surgical, anesthesia, ICU, and heart failure teams.
* Educate patients and families on transplant eligibility, treatment options, immunosuppression adherence, infection precautions, lifestyle changes, recovery expectations, and long-term follow-up requirements.
* Maintain accurate consultation notes, transplant assessments, listing documentation, medication plans, rejection monitoring records, discharge summaries, and follow-up reports in line with hospital standards.
* Contribute to teaching, research, clinical audits, academic activities, publications, and mentoring within the department.
* Apply international transplant guidelines, patient safety standards, ethical practice, and evidence-based cardiac transplant medicine in all clinical decisions.

Ideal Profile

* MBBS, MD, or equivalent medical qualification from a recognized institution.
* Board Certification in Cardiology, Internal Medicine, or an equivalent recognized specialty.
* Subspecialty training in Advanced Heart Failure and Cardiac Transplantation.
* Fellowship from a recognized international cardiac transplant center.
* Minimum 3-5 years of post-fellowship experience in cardiac transplant medicine.
* Strong knowledge of international transplant guidelines, including ISHLT and ACC/AHA standards.
* Experience managing advanced heart failure, transplant candidacy assessment, post-transplant follow-up, immunosuppression therapy, rejection surveillance, and graft monitoring.
* Practical exposure to mechanical circulatory support programs, including LVAD and ECMO assessment.
* Experience in tertiary care hospitals, advanced heart failure units, cardiac transplant centers, or internationally accredited healthcare institutions is highly preferred.
* Strong clinical judgment, communication skills, multidisciplinary teamwork, and patient counseling ability.
* Ability to participate in transplant committees, academic activities, research, quality improvement, and departmental mentoring.
* Commitment to evidence-based medicine, ethical practice, accurate documentation, patient safety, and continuous professional development.
