Role Context

The Consultant - Transplant Critical Care will lead the management of critically ill transplant patients requiring intensive monitoring, organ support, rapid clinical decisions, and coordinated specialist input. The role covers pre-operative optimization, intra-operative critical care coordination, post-operative ICU management, mechanical ventilation, hemodynamic support, renal replacement therapy, invasive monitoring, sepsis management, immunosuppression-related complications, and emergency consultations across critical care areas. The consultant will help strengthen transplant ICU services in Dubai through evidence-based care, strong leadership, ethical practice, and close collaboration with transplant surgeons, nephrologists, hepatologists, anesthetists, emergency physicians, nurses, and allied healthcare teams.

Core Duties

* Lead ICU care for critically ill liver and kidney transplant patients across pre-operative, intra-operative, and post-operative phases.
* Manage complex transplant-related critical care cases involving organ failure, hemodynamic instability, respiratory failure, infection risk, immunosuppression complications, and multi-system deterioration.
* Collaborate with transplant surgeons, nephrologists, hepatologists, anesthetists, intensivists, infectious disease specialists, pharmacists, nurses, and allied healthcare professionals.
* Perform and supervise advanced ICU procedures including invasive monitoring, mechanical ventilation, renal replacement therapy, airway support, vascular access, and hemodynamic optimization.
* Develop individualized critical care plans based on transplant status, organ function, laboratory trends, imaging findings, medication profile, and clinical risk.
* Provide expert support for emergency consultations across ICU, ER, operating theatres, recovery areas, and transplant units.
* Ensure transplant ICU practice follows international best practices, DHA regulations, hospital policies, infection control standards, and patient safety protocols.
* Guide clinical decision-making for ventilator strategies, fluid management, vasopressor support, renal support, sedation, nutrition, sepsis care, and post-operative recovery.
* Monitor critically ill patients closely and adjust treatment plans in response to changing clinical condition, laboratory results, organ function, and multidisciplinary recommendations.
* Maintain accurate clinical documentation, including ICU notes, procedure records, treatment plans, consent discussions, handover details, and discharge or transfer summaries.
* Participate in clinical research, quality improvement initiatives, transplant outcome reviews, morbidity and mortality meetings, and ICU governance activities.
* Train and mentor junior doctors, fellows, ICU nurses, residents, and allied health staff in transplant-related critical care practice.
* Provide on-call support for transplant ICU emergencies, acute deterioration, and complex critical care decision-making.
* Support a patient-centered and family-aware approach by communicating clearly about risks, treatment goals, prognosis, and ongoing care plans.

Ideal Profile

* Medical degree such as MBBS or equivalent from a recognized institution.
* Completed residency in Critical Care Medicine, Internal Medicine, or a closely related specialty.
* Fellowship or subspecialty training in Transplant ICU or Critical Care Medicine.
* Board certification such as UK CCT, American Board, Canadian FRCP, or equivalent recognized qualification.
* Valid license or eligibility for DHA Consultant classification.
* Minimum 5 years of post-fellowship experience in transplant-related ICU care.
* Proven experience managing liver and kidney transplant patients in a tertiary care, transplant center, or advanced ICU setting.
* Strong command of advanced ICU technologies, invasive monitoring systems, mechanical ventilation, renal replacement therapy, and EMR systems.
* Excellent leadership, communication, clinical decision-making, and multidisciplinary coordination skills.
* Ability to manage high-pressure ICU situations with accuracy, calm judgment, and strong ethical standards.
* Proficiency in English is required; Arabic language skills will be an advantage.
* Interest in academic teaching, transplant-related research, quality improvement, or ICU service development will be valued.
