The role covers the entire transplant pathway, beginning with eligibility assessment and continuing through listing decisions, peri-operative management, immunosuppression and long-term graft surveillance.
This position has a direct impact on survival, transplant readiness and quality of life for patients with end-stage cardiac disease. Close coordination with cardiac surgeons, intensivists, anesthesiologists, transplant coordinators and other cardiovascular specialists is essential for safe decision-making and continuity of care.
Key Responsibilities
* Evaluate patients with advanced or refractory heart failure for specialist treatment options.
* Determine suitability for heart transplantation based on clinical status, comorbidities and established selection criteria.
* Conduct comprehensive pre-transplant assessments and participate in listing evaluations.
* Present complex cases to multidisciplinary transplant selection committees.
* Develop individualized management plans for patients awaiting transplantation.
* Provide specialist medical management throughout the peri-operative transplant period.
* Deliver structured post-transplant follow-up and monitor clinical recovery.
* Manage immunosuppressive therapy according to patient condition and transplant protocols.
* Adjust immunosuppression when necessary to balance rejection risk, infection and treatment-related complications.
* Coordinate rejection surveillance using appropriate clinical and diagnostic strategies.
* Identify suspected acute or chronic rejection and initiate timely management.
* Establish infection prevention and monitoring strategies for immunosuppressed patients.
* Monitor long-term cardiac graft function and identify emerging complications.
* Assess candidates for mechanical circulatory support when transplantation is not immediately available or appropriate.

A minimum of 3-5 years of post-fellowship experience in cardiac transplant medicine is expected, ideally within a tertiary-care or high-volume transplant program.
